The Young People’s Positive Pathways Project operates in Denbighshire to prevent homelessness.

Support Workers carry out assessments with any young person (between the age of 16 and 25) presenting to the Local Authority as homeless or at risk of becoming homeless.  As part of the assessment, the Support Worker will identify the support needs of the young person and identify the steps needed to be taken to prevent or address homelessness.  The project works closely with the Housing Solutions Team, Children’s Services, Youth Justice Service and agencies such as Child & Adolescent Mental Health Services.

A key outcome for this project overall will be to reduce the use of temporary accommodation for under 25s and, in general, meet the requirements of the Welsh Government Young Person’s Positive Pathway model.
